- What does a good day look like for my loved one?
Start with the person—not the diagnosis. - What’s missing from their current supports?
Identify gaps early before they become problems. - Are we making progress on their goals?
Request data or examples. - What natural or community supports could be added?
Not everything has to be paid service. - What changes should we expect over the next year?
Transitions, aging, or changing interests should be proactively addressed.
